Rapidly Progressive Glomerulonephritis (RPGN)

Etiology

 • Goodpasture syndrome (anti-GBM)
 • Granulomatosis with polyangiitis (ANCA)
 • Post-infectious or idiopathic

Pathogenesis

 • Crescent formation → rapid loss of renal function

Clinical features

 • Rapid onset oliguria/anuria
 • Hematuria, proteinuria
 • Fatigue, edema

Lab findings

 • ↑ Serum creatinine, ↑ BUN
 • RBC casts
 • Serology: anti-GBM, ANCA

Treatment

 • High-dose steroids + cyclophosphamide
 • Plasmapheresis for anti-GBM
